The packages for General Accounting Office, Senate, and House of
Representatives each contain two copies of the transmittal form (one original
and one copy). Both the original and the copy are tagged with a “Sign
Here” tag at the bottom. Please have the person at the desk who receives
the package sign and date both the original and the copy.
After signing, the recipient keeps the original. You keep the signed
copy and return it to the originating office at FWS. The signed copy
